List of new Lutheran Health Physicians providers grows

FORT WAYNE, Indiana (Sept. 7, 2017) – Two nurse practitioners and a podiatrist have recently joined Lutheran Health Physicians.

Kristin Toliver, DPM, is a podiatrist who cares for patients in need of medical and surgical treatment for disorders, diseases and injuries of the feet and ankles. As a podiatrist, Dr. Toliver diagnoses, treats, and prescribes medication to patients and oftentimes detects undiagnosed health problems, such as diabetes, arthritis and heart disease.

Dr. Toliver earned her Doctor of Podiatric Medicine degree from the Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science in North Chicago, Illinois. She served as chief resident during her podiatric medicine and surgery residency at Katherine Shaw Bethea Hospital in Dixon, Illinois.

Dr. Toliver joined Lutheran Health Physicians after working as a podiatrist at the Family Foot Center Podiatry Group in Southern California. She is a member of the American College of Foot and Ankle Surgeons, the American Podiatric Medical Association and the American Society of Podiatric Surgeons.

Dr. Toliver is based on the first floor at 303 S. Main St., in Bluffton.

Neil Shorb, NP, is a nurse practitioner who treats patients with severe, life-threatening blunt force and penetrating injuries. As a member of the Lutheran Health Physicians trauma surgery team, Shorb assists surgeons as they operate on patients who are critically injured and oftentimes suffering from multiple injuries to the colon, pancreas, spleen and/or stomach.

Shorb graduated from Bethel College in Mishawaka with a bachelor’s degree in nursing. He earned his Master of Science in Nursing degree and family nurse practitioner certification from the University of Saint Francis in Fort Wayne.

Shorb previously worked as a registered nurse at Lutheran Life Villages in Fort Wayne providing acute and special needs care for residents. He also cared for critically ill patients in the intensive care unit at Dupont Hospital.

Shorb is a member of the Coalition of Advanced Practice Nurses.

He is based in Medical Office Building One, 7900 W. Jefferson Blvd., Suite 306, on the Lutheran Hospital campus.

Holly Young-Reese, FNP, a board-certified family nurse practitioner, is a member of the Lutheran Health Physicians cardiovascular/thoracic surgery team. She primarily assists physicians performing surgeries on the heart, lungs and major blood vessels – and sometimes the esophagus, sternum and trachea.

Prior to joining LHP, Young-Reese served as a family nurse practitioner providing patients with orthopedic spine care. She also worked for seven years as a registered nurse at St. Joseph Hospital caring for patients in preoperative and recovery units, and in the outpatient unit at Inverness Surgical Center.

Young-Reese earned her bachelor’s degree from Ball State University in 1993. She graduated from the University of Saint Francis with a Master of Science degree in nursing and a family nurse practitioner certification in 2013. She is a member of the Coalition of Advanced Practice Nurses of Indiana, the American Academy of Nursing, Sigma Theta Tau International Honor Society of Nursing and the American Nursing Credentialing Center.

Young-Reese is based out of Medical Office Building Two, 7910 W. Jefferson Blvd., Suite 102, on the Lutheran Hospital campus.
